2025 Request a Certificate of Participation

Please complete the fields below and a certificate of participation will be e-mailed to you following the 2025 Annual Meeting. You must register and attend the conference to receive a certificate. Processing time may take up to 2 weeks following the annual meeting.

Certificate of Participation requests must be received by August 15, 2025.


If you require certification to be sent to a University Official, please provide the full name and e-mail address where it should be sent below. It is your responsibility to provide accurate information.
